Introduction

Clark Quality Care, LLC provides residential group home services for children and adolescents with emotional, behavioral, or developmental needs. By enrolling a child in our program, guardians and referring agencies agree to comply with these Terms and Conditions.

Program Admission

– Admission is subject to referral, assessment, and acceptance by Clark Quality Care, LLC.
– Guardians and agencies must provide complete and accurate medical, behavioral, and educational history.
– False or incomplete information may result in denial or termination of services.

Services Provided

Our program offers:
– 24/7 supervised residential care
– Individualized care plans
– Therapeutic support and counseling
– Educational and life skills development
– Community integration and family engagement opportunities

Resident Rights

All children in our care have the right to:
– A safe, supportive, and nurturing environment
– Respect, dignity, and privacy (within program guidelines)
– Access to education, medical care, and therapeutic support
– Protection against neglect, abuse, or exploitation

Guardian/Agency Responsibilities

– Cooperate with staff and provide necessary documentation and consents.
– Attend scheduled family meetings, therapy sessions, and progress updates.
– Ensure that financial obligations (if applicable) are met in a timely manner.

Confidentiality

– All resident records and personal information are strictly confidential.
– Information will only be shared with authorized individuals or as required by law.
– Guardians must respect the confidentiality of other residents and families.

Safety and Conduct

– Residents must follow house rules to ensure safety and harmony.
– Prohibited behaviors include violence, possession of contraband, and endangering others.
– Staff reserve the right to implement safety protocols, including behavior management interventions, when necessary.

Medical Care

– Clark Quality Care, LLC will coordinate routine and emergency medical care.
– Guardians must provide up-to-date medical records and consent for treatment.
– Any costs not covered by insurance or state programs may be the guardian’s responsibility.

Termination of Services

– Services may be terminated if the resident no longer meets program criteria, poses a serious risk to others, or if guardians/agencies fail to comply with obligations.
– Notice of termination will be provided, except in emergencies where immediate discharge is necessary for safety.

Liability

– While Clark Quality Care, LLC provides supervision and therapeutic support, the organization is not liable for unforeseen incidents beyond reasonable control.
– Guardians and agencies agree to hold Clark Quality Care, LLC harmless against claims arising from willful misconduct or non-disclosure of critical information.

Amendments

Clark Quality Care, LLC reserves the right to update these Terms and Conditions. Guardians and agencies will be notified of any significant changes.
