Resident Rights & Responsibilities Agreement

Clark Quality Care, LLC is committed to protecting the rights, dignity, privacy, and safety of every resident. This agreement outlines the rights and responsibilities of the resident and/or their authorized representative.

Resident Rights

• To be treated with dignity, respect, and fairness at all times.
• To live in a safe, clean, and supportive environment.
• To receive services in accordance with the Individual Service Plan (ISP).
• To participate in decisions regarding care, services, and daily activities.
• To communicate privately and freely with family, guardians, advocates, and legal representatives.
• To have personal privacy and confidentiality of records maintained.
• To practice religious beliefs and cultural traditions of choice.
• To voice concerns, complaints, or grievances without fear of retaliation.
• To be free from abuse, neglect, exploitation, or unnecessary restraint.
• To access personal belongings unless restricted for documented safety reasons.

Resident Responsibilities

• To treat staff, peers, and visitors with respect.
• To follow house rules, safety guidelines, and program expectations.
• To participate in assigned services, programs, and activities as outlined in the ISP.
• To respect the rights, privacy, and property of others.
• To report safety concerns, injuries, or issues to staff promptly.
• To use equipment, furnishings, and property responsibly.

Acknowledgement

By signing below, the resident and/or authorized representative acknowledges that they have received, read, and understand the Resident Rights & Responsibilities and agree to comply with this policy while services are being provided by Clark Quality Care, LLC.
